Acute pulmonary embolism (PE), particularly intermediate high-risk PE, remains a major clinical challenge due to the uncertainty surrounding optimal management strategies that balance efficacy and safety. Although systemic anticoagulation is standard of care, patients with intermediate high-risk PE face a substantial risk of early clinical deterioration, hemodynamic collapse, and PE-related mortality. The HI-PEITHO trial was designed to address a critical evidence gap by comparing standard anticoagulation alone with anticoagulation plus ultrasound facilitated, catheter directed, low dose thrombolysis. By evaluating a composite endpoint of PE-related mortality, cardiorespiratory decompensation, and recurrent PE within seven days, the study directly targets clinically meaningful outcomes that influence early decision making in high acuity settings.
