Manuscript - Safety and Feasibility of the RevCore Catheter for Venous In-Stent Thrombosis: A Multicenter, Retrospective Study

This research analyzes the safety and feasibility of the RevCore thrombectomy catheter, a device by Inari Medical, used to treat venous in-stent thrombosis (IST). Following venous stent placement, patients often experience thrombosis within the stent, which is challenging to treat effectively. Traditional therapies, like venoplasty, are often inadequate at maintaining stent patency, and fibrinolytic therapies carry a high risk of major bleeding.<br /><br />In this multicenter, retrospective study, the RevCore device, which is specifically designed to treat IST, was evaluated across 44 patients (48 treated limbs) from four centers. The study aimed first to achieve technical success, defined as achieving an effective stent diameter of less than 50% post-procedure as assessed via intravascular ultrasound. Safety endpoints included adverse events such as mortality, readmissions, significant pulmonary embolisms, and vessel perforation within 30 days.<br /><br />Results demonstrated that the primary technical success was achieved in all of the treated limbs, with the effective stent diameter significantly improving from 21.1% to 89.6%. Importantly, no patients experienced any of the predefined serious adverse events. At the first follow-up, conducted at a median of 40 days post-procedure, primary stent patency was achieved in 94% of limbs.<br /><br />The study also found that patients with severe venous disease (classified as C6 on the Clinical-Etiology-Anatomy-Pathophysiology scale) showed significant improvements post-treatment. Overall, the RevCore catheter effectively reestablished and maintained patency in treated veins, indicating a potentially beneficial therapy for those with venous IST.<br /><br />This research suggests that the RevCore catheter is safe and effective in dealing with IST, offering promising results where conventional therapies may fall short. Future prospective studies will further elucidate these findings and implications for clinical applications.
